Job Description:
A company is looking for a Community Well Care Coordinator for
remote work in Indiana.
Key Responsibilities:
Assist with care management for high medical risk members with
comorbid behavioral health needs
Conduct comprehensive needs assessments and develop individualized,
person-centered care plans
Collaborate with internal care teams, providers, and community
resources to implement care plans
Required Qualifications:
Bachelor's degree in social work, psychology, counseling,
gerontology nursing, or health & human services, or equivalent
experience
1+ years of experience with MS Office applications
Access to a designated quiet workspace at home for secure handling
of Protected Health Information (PHI)
Must reside in Indiana and have access to high-speed internet
Minimum of two years of direct service experience with older adults
or individuals with disabilities preferred
