Job Description:

A company is looking for a Care Manager RN, OB/Maternity.

Key Responsibilities

Advocates for and educates members on services and self-management techniques
Conducts assessments to identify barriers and develop interdisciplinary care plans
Collaborates with healthcare providers and community resources to coordinate care

Required Qualifications

Current New York State Registered Nurse (RN) license
Experience in care management and working with childbearing individuals and families
Strong interpersonal and assessment skills
Fluency in Spanish, Korean, Mandarin, or Cantonese is preferred
Relevant work experience, preferably as a Care Manager
