About the Role: We are looking for a compassionate and skilled Palliative Care Registered Nurse to join our team. As part of the Advanced Illness Management program, you will work closely with patients, their families, and healthcare professionals to provide high-quality end-of-life care. This role involves in-person and telephonic assessments, patient education, and coordination with the interdisciplinary team. Responsibilities:

• Conduct in-person/telephonic RN assessments and provide education to palliative care patients and their families.
• Complete Risk Stratification Assessments and identify patient support needs in consultation with the NP.
• Educate patients and families on disease processes, self-care techniques, end-of-life care, medications, and nutrition.
• Coordinate care with the palliative team members and support care plan development and implementation.
• Facilitate in providing and/or obtaining care for patients when a need is identified. Requirements:
  • Registered Nurse license to practice in the state.
  • 2+ years of prior skilled Registered Nurse experience.
  • Experience in palliative care, oncology, hospice, or geriatric care preferred. Benefits:
